They're just bigger forms that have a greater chance of dropping a red item.

They arent that hard to see, I normally see one in every 2 forest runs at least.

They drop the same stuff as the normal el rappies, they just have an increased chance of finding it.

I finally got ultimate mode today, and i found one of those. I got ultimate aat lv 77.

From a Forest El Rappy, you get a Red Handgun. Not bad .
