Umm.....what determines rares in boxes, you ask? Well..to be honest, I'm not 110% sure. But there is just a certain chance that they will appear in the boxes in the rares' designated area.
(btw, i dont believe that the drop charts @ faqs.ign.com have the box drop charts)

and yeah ive only seen one rare monster... I like the experience it gives from its normal counter-parts tho. I wish bosses would drop rares 100% of the time, it'd make getting an item you want easier...
Telepiping is fun when the first rare, in my case a Nar Lily, pops up after a few minutes, killing it and getting a rare item afterwards telepiping again and getting a rare enemy after the first pipe again. Those were 2 easy God/Minds.

ya well maybe but i didn't use any telepies at all or go through the transporter, i was doing "Forrest of Sorrow" quest (don't remember difficulty) and i had just killed one hideblue(the last one in the room) and walked straight into another room and there stood another hideblue.
ps sorry i meant those rare mech guns that were used by the Justice Corp. not a justy(i thought it was there nickname)

before my game got corrupted my Racast found a Arms+2 with a 90% Native and i think a 35%A.Beast and a 20%Machine.
actually the chances are like 1/11 and aboout 1/30 in hard mode not that rare m8
I lost my last Yamato to a corrupted game (word to the wise: make sure you've moved your info back onto the card when you're playing at a buddy's house before you turn off the Cube). Shouldn't be any big thing to get another one once I rebuild some inventory.
