In ultimate they all change allthough they have the same attacks they also change names.

The difference between the first three is just the difficulty of the enemies. More HP, more attack power etc. They also drop more rare (not more of them, more rare rares :p) items.
The difference between V.Hard and Ultimate is very noticeable. Most of the monsters have changed appearance and I think the difficulty shoots up (hence the name

How do you actually change the difficulty though?
Is it automatic?
If so, how can you find out which difficulty level you are playing on?

if you're offline, after choosing a character you select the diffuculty.
If you're online, you join/create a team of whichever difficulty you like.

Yes, as decswxaqz said, Normal through Very Hard they look the same, only they have more Hp, more exp.
If your offline, you have to beat normal to get to hard, hard to very hard, and so on.
Online or Multilplayer:
1 - Normal
20 - Hard
40 - V. Hard
80 - Ultimate
