it depends on drop rates, usually, Hard and Very Hard have better drop rates, but Ultimate has better stuff, hence rarer.

Also, when I've found like the specials off of a special monster, I've found that once I have 2 in my inventory they stop dropping the item. Is there a reason for that or is it just bad luck? :'(
