There is nothing on his left, the only other thing on him is his equipped Zanba. And yes, you get it from Paganii, if you land on Nol she'll give you a special wepon, usually with Hit. I got a Gae-Bolg, and a DB's Saber with 20% Hit. And that is where I got my CrossScars with 70% Hit.
