you can change many creature parts into weps offline.
complete the quests its easier that way then go on unsealed door go out of the guild and dr montogue will be ahead of you to the left talk to him and show him the item you want changing.

Lv45 means you have 346 HP if you haven't used any materials. So You'll need about 40 extra HP. (1 Dragon/HP or 4 Digger/HPs) Attack wise you should be fine.
