It's not a glitch at all. It's just ST's way of making the game a little more challenging. If you do a level repeatedly for a rare though, you'll eventually memorize all of the maps. There are also certain rooms that every map of a level has (ex: Cave1 always has the long recantanglish room with the whole in the middle).
The only level that a map is really useful for is Ruins because it's so huge and there are so many detours.
The quests also use the layouts of regular level maps. They just may +/- enemies and throw in an extra switch or two.
The PSOW maps are very strange I've found. Some days they work, some days they don't. It could just be my computer, but the PDF has always worked for me. It may not be working right now because PSOW was recently closed down for a while (I have no idea why).
