Apparently, it can drop from any monster in Parum or Moatoob, provided the monsters are at least level 20.

It's dropped for me on Neudaiz as well... so I imagine it can drop anywhere.

That sounds about right.
In addition, it's listed as a Special Drop for the Darbelan rare spawn.
