Hello, FOmar. 
For the last form, you can't dodge the Grants attack, so if you are getting killed by it, make sure you are using 4-slotted armour, especially the one which gives light resistance (I hope I remembered this right; the DFP defense is not important for Dark Falz because his "physicals" do a set damage) and put on as many Resist/Holy, or more likely, Resist/Saint from the shop as you can get. If you still can't survive with a full HP bar, then you need more levels.
For attacking, unless you're a melee FOmar, use a mind MAG and Foie to kill him quickly with Club of Laconium. Good luck then.
